How to Register
Registration consists of three components. Please complete all steps before the deadline.

Registration Form
Complete the registration form to sign up for Math League. Make sure all information is accurate before submitting.

Signature Form
Both the student and parent must sign the signature form. Place the signed form in an envelope and turn it in to the Math League box in the CVMS front office by September 3rd, 2025.

Suggested Donation
A suggested donation of $50. Donate online through CVMS PTSA, or via check made out to CVMS PTSA with a note to Math League and your student's full name. If paying by check, include it in the same envelope as your signature form.
